This question is to accurately identify alumni records in our database, regardless of any artist, stage, or maiden names. Our goal is to ensure we have your correct and preferred name on file, as we respect your identity and strive to maintain accurate records. 

Please note this form will not update your primary name across Berklee's systems and campuses, please use the Update Name Change form to initiate a name update.
